The management of cerebral aneurysms and arterio-venous malformations (AVMs) still poses considerable problems and is still the subject of controversy. In spite of dramatic advances in surgical techniques, endovascular therapy and anesthesia, the largest lesion – as well as those located in deep areas of the brain – remain a challenge for the neurosurgeon. When dealing with aneurysms, the initial sub-arachnoid hemorrhage can be devastating – creating difficulties in management – and cerebral vasospasm still has to be solved. The hemodynamic consequences of the occlusion of large AVMs are difficult to predict and combined or alternative treatments may be preferable to surgical removal of these lesions. For less common malformations – such as dural fistulas, cavernomas and venous angiomas – the indications for treatment have still to be established. In this volume international experts discuss in depth the various aspects of these pathologies, including diagnosis, pathophysiology, pharmacology, surgery, embolization and endovascular occlusion, radiosurgery, anesthesia and perioperative care.
